Appointments

Corporate Members and private patients may schedule appointments online or by speaking with a Resource Director at 800.362.8671.

As a Corporate Member, your examination protocol has been pre-determined based on your age and gender. Should additional exam components within our scope of services be recommended based on your physician's exam findings or as a result of your medical history, these will be added at the time of exam. To make an appointment online, you will need your previously issued Unique User ID and Password (see Program material mailed to your home). If you don't have this information, please email us at Assistance@EHEINTL.com, or call a Resource Director.

Private patients too can schedule online an annual examination, cardiac stress test, vascular screening, colonoscopy, mammogram, flu vaccination, and travel medicine services. You will be asked for credit card information for an appointment confirmation (we accept Visa, Mastercard, American Express, and Discover Card). Your card will not be charged until the date of service.

Medical History

Did you know that a complete, accurate Medical History is the cornerstone to a comprehensive preventive health program? It's your physician's guide to their assessment of your total health. Once you've scheduled your appointment, you will be asked to complete your Medical History at least seven (7) days prior to that date. You may complete your Medical History online in a secure environment, or request a paper form.

Returning Corporate Members and private patients who previously completed an online Medical History need only update online their personal health information!
