Privacy Policy

Please read the following to understand EHE International's views and practices regarding privacy and how they pertain to users of Executive Health Exams International's websites.

EHE International ("EHE") and its technical subsidiary, PersonalMD, are strongly committed to protecting the privacy of patients and members (also referred to as "Users" in this Privacy Policy) who use its interactive products and services. At EHE, we understand that your health information is very personal and private. EHE has created this privacy statement in order to demonstrate our firm commitment to you and your privacy. Our service permits patients to store and access their medical information in a safe and secure place. Our business model is not dependent on selling any individual's personal identifiable information. The following discloses our information gathering and dissemination practices for this website located at www.ExecutiveHealthExams.com and www.PersonalMD.com (also referred to in this Privacy Policy as the "Sites"), how we collect the information, and what choices you have about how that information is used. We urge you to read the Privacy Policy so that you will understand both our commitment to you and your privacy, and how you can participate in that commitment.

EHE may, from time to time, change the features and content found on its Sites. For example, regulations are expected to be issued under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). The HIPAA regulations are expected to contain rules governing the use, storage and disclosure of protected health information stored or transmitted in electronic form. Consequently, this Privacy Policy will be modified to reflect the HIPAA regulations when issued in final form, and may change from time to time to reflect any new features or content. Please revisit our Privacy Policy from time to time to be sure that you are familiar with our current policy.

As indicated in EHE's Terms of Service for the Sites, your use and/or continued use of the Sites (following any modifications to this Privacy Policy) will be conclusively deemed acceptance of this Privacy Policy and any changes thereto and will constitute consent to EHE's collection and use of your personal information as outlined in this Privacy Policy. Please see EHE's Terms of Service for more information on the terms and conditions governing the use of the Sites. If you do not agree to the terms of this Privacy Policy, please exit the Sites immediately.

Why does EHE need my personal information? EHE uses personal information primarily for your use with an Online Health Record. The Online Health Record is a service that allows you to access and inspect your medical records collected in connection with your medical examination conducted at EHE- affiliated medical practice offices or entered yourself via a registration process available at www.PersonalMD.com. You may also choose to input, via facsimile, medical data that you have obtained outside of your medical examination conducted at EHE-affiliated medical practice offices. Additionally, you will have the ability to update some of the medical information contained within your Online Health Record.The Online Health Record is designed to allow access only by means of a valid password that you create.

In order to provide you with a more personalized and relevant web service, EHE also collects and uses personal information to identify your specific interests and needs.

How and what information does EHE gather or track? Through the use of on-line data entry, survey, and registration forms, EHE may collect and/or monitor, in aggregate and individual form, personal identifying facts or data on, among other things, your e-mail address. EHE obtains most of the personal information from patient records gathered at our affiliated practice offices in connection with the operation of Online Health Records.

Other information may be collected during the initial registration process to access these Sites. Although some of the personal identifying information is necessary for EHE to deliver its interactive products and services, some of the information collected is optional with respect to use of the Sites. Accordingly, you can determine for yourself, to some extent, what information you wish to submit on a voluntary basis to EHE. In addition to the initial registration process, EHE occasionally asks you for personal information at other times, including when you complete optional survey forms or fill out optional contact forms for additional products and services. We also collect information about you and your use of our Sites through the use of "cookies."

What are Cookies? "Cookies" are small pieces of information that are stored by your web browser software on your computer's hard drive or temporarily in your computer's memory. The use of cookies is now an industry standard, and you will find them used by most major web sites. EHE may place and store Internet cookies on your hard drive. Cookies can save any of the types of information noted above. Cookies enable EHE to personalize your experience on the Sites. When you revisit the Sites, EHE can recognize you by the cookie and customize your experience at the Sites accordingly. For example, EHE uses cookies to recognize access privileges to the Sites, track our Sites usage and traffic patterns, add advertisements for goods or services of interest, and estimate the Sites audience size. EHE also uses cookies to store your User name and password. We use this research to understand how our Users' habits are similar or different from one another so that we can make each new experience on EHE a better one. We may use this information to better personalize the content, banners, and promotions that you and other Users will see on our sites. Although most web borrowers are initially set up to accept cookies, if you prefer, you may decline the placement of a cookie on your hard drive by using the appropriate feature(s) of your web browser software (if available) to delete the cookie. You should understand, however, that certain areas within the Sites may not function properly if the web browser will not accept cookies. Advertising networks that serve ads onto EHE may also use their own cookies.

Does EHE disclose personal information to third parties? Under no circumstances will EHE release any of your personal identifiable information stored in Online Health Records to any person or company without your consent, unless otherwise required by law or ordered by a court of law. We use state-of-the-art technology to ensure that your information can be stored and retrieved securely and safely. However, you are responsible for taking all reasonable steps to ensure that no unauthorized person shall have access to your Online Health Records password or account. It is your sole responsibility to (1) control the dissemination and use of activation codes and passwords; (2) authorize, monitor, and control access to and use of your Online Health Records account and password; and (3) promptly inform EHE of any need to deactivate a compromised password. You grant EHE and all other persons or entities involved in the operation of the EHE Sites and Online Health Records the right to receive, transmit, monitor, retrieve, store and use your personal identifiable information, including information that may be privileged and confidential under applicable state and federal laws, in connection with the operation of Online Health Records. EHE does not assume any responsibility or liability for any information you submit to Online Health Records or your or any third parties' use or misuse of information transmitted or received using Online Health Records.

EHE may disclose de-identified personal information in aggregate form to third parties or advertisers. This de-identified personal information will be aggregated and disclosed in whole from EHEs entire User database. For example, EHE might tell an advertiser how many Users visited the Site over a period of time. EHE does not disclose individual personal information records to third parties without your prior consent, unless compelled to do so by applicable law or valid legal process. In the future, if EHE deals with third party advertisers or retailers offering products or services which EHE believes may be of interest to you, EHE is not and cannot be responsible for the activities or privacy policies of these third parties.

What about external web sites? The Site is managed by EHEs technical subsidiary, PersonalMD. To the extent hyperlinks or banner advertisements incorporating hyperlinks are utilized to access third party sites, you should be aware that these third party websites are not controlled by EHE and, therefore, are not subject to this Privacy Policy. You should check the privacy policies of these individual sites to see how your personal information will be utilized by the proprietors of those third party web sites.

What does EHE do to keep the Site secure? PersonalMD, subscribes to HON (Health on the Net) ethical guidelines, which ensure confidentiality of all health information on the Internet. Information stored in Online Health Records is protected using state-of-the-art encryption technology. No one, except you and physicians you authorize, can access the encrypted personal information.

EHE has implemented a number of security features throughout the Site to prevent the unauthorized release of or access to the personal information it collects through the Sites. For example, database information is kept separate from live servers and is protected by two firewalls. Also, only authorized personnel have access to any stored personal information. Only you and those to whom you have provided written authorization may have access to your medical information.

Please be advised, however, that although EHE has endeavored to create a secure and reliable site for you, the confidentiality of any communication or material transmitted to/from EHE via the Sites or e-mail cannot be guaranteed. When disclosing any personal information, you should remain mindful of the fact that it is potentially accessible to the public, and consequently, can be potentially collected and used by others without your consent. You should therefore consider carefully if you want to submit sensitive information that you would not want disclosed to the public. You should also recognize that your use of the Internet and the Sites is solely at your own risk. You agree that EHE has no responsibility or liability for the security of personal information transmitted via the Internet. EHE, however, urges you to exercise caution when conveying personal information over the Internet.
